Accident/Injury First Report
Employees are required to fill out an Accident/Injury First Report (PDF) anytime there is an accident causing damage to property or injury to person involving a City employee or occuring during a City sponsored event, while using City vehicles or equipment, or on City property. Reports are required within 24-48 hours after an accident occurs. Please follow the Accident and Injury Flow Chart (PDF) for employee injuries.
